Applying For Your CSOS Certificate

To obtain your CSOS certificate, go to the DEA eCommerce website and follow these instructions:

1. Determine which of your personnel will need to sign electronic orders for controlled substances. Each person who will place orders must acquire his or her own certificate.
2. Click the Just Getting Started box on the DEA website home page.
3. Choose your application type. Most applicants will enroll as registrants.
4. Read the three CSOS enrollment documents provided on the site and click the “I accept and continue” button at the bottom of each screen.
5. Click on the application form that appears. For example, if you enroll as a registrant, you’ll receive the Form 251 application. Follow the instructions for printing the form and completing the application.
6. Designate a principal coordinator on your application. If that person is not the registrant, a separate principal coordinator application must be completed by going back to www.deaecom.gov and repeating the process for the appropriate application.
7. If you apply as registrant for more than one DEA registered location, you must also complete the CSOS Certificate Application Registrant List Addendum.
8. Sign and notarize your completed application and addendum(s), and follow the DEA’s mailing and certificate retrieval instructions.
